Position Overview

We are seeking a dedicated and proactive Senior Clinical Practitioner to become a vital part of our innovative Acute Home Visiting Service.

This role will also encompass responsibilities within the Community Support Beds, enhancing the diversity of your clinical practice.

As a Senior Clinical Practitioner, you will operate independently, managing a broad spectrum of patient demographics.

You will play a crucial role within the multi-disciplinary team (MDT), collaborating closely with colleagues to deliver exceptional care.

Our initiative, Team Up, integrates community services across healthcare, social care, and voluntary sectors, ensuring comprehensive support for patients who are housebound.

In this capacity, you will work alongside general practitioners, significantly impacting the success of this new service.

Additionally, you will have the chance to take on a leadership role within a modern facility, providing care in the Community Support Beds.

Key Responsibilities

Your primary duties will include:

  • Delivering clinical care to residents at the Community Support Beds, while overseeing the MDT.
  • Supporting the Home Visiting Service by collaborating with GPs to assess referrals, conduct home visits, and provide tailored care for housebound patients.
  • Exercising a high level of autonomy in clinical decision-making.
  • Demonstrating excellent communication and interpersonal skills to foster effective relationships.
  • Exhibiting confidence and adaptability in clinical practice, utilizing independent prescribing and clinical guidelines.
  • Contributing to the formulation of relevant policies and protocols, driving service enhancements.
  • Traveling to provide care to housebound patients as necessary.
  • Being available for evening and weekend shifts as required.

A Band 8a position will be available for candidates who possess an MSc in Advanced Practice (or equivalent) and can demonstrate relevant advanced clinical experience.

We employ approximately 4,200 staff members, providing care in various settings, including community hospitals, health centers, clinics, GP practices, schools, care homes, and increasingly, in patients' homes and through virtual consultations.

During the pandemic, we successfully led the establishment of public vaccination centers in collaboration with health and social care partners.

Job Requirements

Essential Qualifications:

  • Professional registration with NMC or HCPC.
  • Evidence of post-registration education at the undergraduate level.
  • MSc in Advanced Clinical Practice or equivalent.

Experience and Knowledge:

  • Experience in a leadership capacity.
  • Contemporary clinical experience with older adults facing complex health challenges.
  • Participation in a multi-professional team environment.
  • Practical understanding of Primary Care and Community Services.

Skills and Attributes:

  • Ability to manage a diverse caseload effectively.
  • Proficiency in MS Office and electronic patient record systems.
  • Exceptional communication and interpersonal skills.
