Key Responsibilities:

  • Deliver comprehensive nursing care to patients in their own homes, focusing on symptom management, pain control, and emotional support.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.
  • Provide education and support to patients, families, and caregivers on palliative care, end-of-life care, and bereavement.
  • Contribute to the development and implementation of service improvements, ensuring that our care is responsive to the needs of our patients and their families.
  • Participate in quality improvement initiatives, audits, and research studies to enhance the quality of our care.
