We are seeking a highly motivated and experienced Registered Mental Health Nurse to join our specialist Early Intervention in Psychosis Team as a Care Coordinator for individuals experiencing a first episode of psychosis.
Key Responsibilities- Maintain and coordinate timely and appropriate interventions through mental health nursing for a caseload of service users living in the Bolton area.
- Work within the framework of the Care Programme Approach (CPA) and in line with relevant clinical pathways.
- Establish good working relationships with a range of providers, including GPs, and participate in multi-agency clinical meetings.
- Conduct first-line assessments for individuals newly referred into the service, utilizing a biopsychosocial framework.
- Registered professional qualification in Mental Health Nursing (RMN).
- Registration with the regulatory body NMC.
- Experience of working within a multidisciplinary team and applying the Care Programme Approach.
- Ability to act as a care coordinator under the CPA framework and establish good working relationships with a range of providers.
