Psychosis Care Coordination Specialist

Leamington Spa, Warwickshire, United Kingdom Coventry and Warwickshire Partnership Trust Full time

Position Overview

Care Coordination Specialist

Band 6

Early Intervention in Psychosis Division

Full-time commitment.

We are seeking a dedicated Band 6 Nurse, Occupational Therapist, or Social Worker to become a vital member of our established Early Intervention team as a Care Coordination Specialist. Our focus is on supporting individuals encountering their initial experience of psychosis.

The Early Intervention in Psychosis model is both innovative and grounded in evidence-based practices. Our teams engage with service users aged 14 to 65 who are facing their first episode or early signs of psychosis, along with their families, for a duration of up to three years. We also assist individuals presenting with at-risk mental states. Our commitment to delivering exceptional care standards is unwavering, and we continuously strive for improvement in our services.

The implementation of the NHS England Access and Waiting Time Standard has catalyzed significant advancements within our team. We are actively providing specialized skills training to clinicians to enhance both client recovery and the attainment of these objectives.

Key Responsibilities

The role encompasses a comprehensive initial evaluation, which includes the CAARMs assessment to identify at-risk mental states (ARMS) or first episode psychosis (FEP). This assessment is integrated into a formulation that guides the client's care plan over the subsequent three years. Our team employs a Recovery-oriented philosophy to collaboratively establish the individual's goals and needs, supporting them in achieving these aspirations. Many of our clients face challenges related to housing, financial stability, vocational pursuits, and social integration. The successful candidate will be expected to work creatively, both independently and alongside team members, to assist clients in reaching their objectives. Our aim is to empower individuals towards independence, reducing their reliance on Mental Health Services.

Additional Information

At Coventry and Warwickshire Partnership NHS Trust (CWPT), we provide a diverse array of services encompassing physical health, mental health, learning disabilities, and autism, proudly serving communities across Coventry, Warwickshire, and beyond.

Our core philosophy is centered around 'people at our heart'; this principle guides our actions and interactions. We prioritize the well-being of our staff and colleagues, offering a comprehensive range of benefits and professional development opportunities.

These benefits include:

  • Generous annual leave that increases with tenure
  • Exceptional learning and development opportunities, including apprenticeship frameworks, distance learning, internal training, coaching, and mentoring
  • Salary sacrifice schemes for vehicle leasing, Cycle to Work, home and electronics, gym memberships, and more
  • Discounts with various retailers, restaurants, and entertainment venues through our Employee Assistance Programme and NHS discount schemes
  • Well-being support, including an in-house counseling service and external helplines
  • Staff networks and support groups

We are continually seeking individuals who share our commitment to enhancing the lives and well-being of our community members, as well as our values of compassion, collaboration, excellence, integrity, and respect.

Job Requirements

Candidates should possess significant experience in community care coordination and demonstrate a passion for promoting holistic health and well-being. A positive and enthusiastic attitude is essential, especially in a demanding role. The ability to work autonomously and collaboratively within a team is crucial, along with effective workload management and diary structuring tailored to client needs. A strong understanding of the Care Programme Approach (CPA) is required, along with proficient IT skills. A key aspect of this role involves risk identification and management, utilizing tools such as the Steve Morgan framework, including positive risk-taking and STORM assessments. The successful candidate will also be expected to perform nursing duties as outlined in the job description.

A valid driving license and access to a vehicle for business purposes are essential for this position.
