Job Title: Adult Care Worker

We are seeking a dedicated and compassionate Adult Care Worker to join our team at Brook Street Social Care. As a key member of our team, you will play a vital role in supporting young adults with their daily lives, helping them to integrate into their local communities and develop essential life skills.

Key Responsibilities:

  • Provide emotional support and guidance to young adults, helping them to overcome challenges and achieve their goals.
  • Assist with budgeting, education, and employment, ensuring that young adults have the skills and knowledge they need to succeed.
  • Work closely with young adults to develop personalized support plans, tailored to their unique needs and goals.
  • Collaborate with other professionals, such as social workers and healthcare providers, to ensure that young adults receive comprehensive support.
  • Conduct regular reviews and assessments to monitor progress and make adjustments to support plans as needed.

Requirements:

  • A minimum of 6 months experience in working within health care or social care sectors.
  • A driver's license and vehicle ownership is essential.
