About the Role

We are seeking a highly skilled and experienced Mental Health Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a Primary Care Mental Health Specialist, you will be working in a dynamic and supportive environment, providing high-quality care to patients with mental health needs.

Key Responsibilities
  • Provide mental health assessments and interventions to patients in primary care settings
  • Work collaboratively with GPs, nurses, and other healthcare professionals to deliver holistic health and social care outcomes
  • Develop and implement effective treatment plans, taking into account the individual needs and circumstances of each patient
  • Conduct risk assessments and manage clinical risk, ensuring the safety and well-being of patients
  • Provide education and support to patients, families, and carers, promoting self-management and empowerment
Requirements
  • Current professional registration as a Registered Mental Health Nurse, Social Worker, or Occupational Therapist
  • Minimum 5 years post-registration experience in working with people with mental health needs
  • Community experience in mental health services for adults and older adults with severe/significant mental illness
  • Experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness
  • Considerable experience in the assessment and management of clinical risk, physical health assessment for people with mental health needs, and effective collaborative working
