We are a domiciliary care provider that offers customized care and support to clients within the comfort of their own homes.

We have transitioned our registration with the Care Quality Commission (CQC) to deliver Total Dependency Domiciliary Intervention (TDDI) and more complex care delivery.

Job Summary:

We are seeking a skilled Registered Nurse to join our team as a Nurse Care Coordinator. The successful candidate will work in the community, providing high-quality care to clients and building customized care plans.

Key Responsibilities:

  • Conduct initial patient assessments, monitor vital signs, symptoms, and well-being, and evaluate changes to care or medical requirements.
  • Carry out a full range of registered nurse duties, including assessment, planning, and implementation and evaluation of the patient's condition and care, with support from staff members in service delivery.
  • Show compassion, treat patients with respect, maintain their privacy, dignity, and be respectful.
  • Demonstrate procedures, train, and supervise staff undertaking care/nurse training.
  • Prioritize nursing care, recognize deterioration, changes, reactions, conduct physical exams to assess patients/any other concerns in patients' conditions, and alert senior staff/health professionals immediately.
  • Perform wound care, ensuring any wounds are cleaned, and dressed as required, along with any other medical requirements/treatments.
  • Comply with infection control policies and procedures.
  • Take patients' temperatures, blood pressure, blood samples, set up drips, and blood transfusions if required.
  • Prescribe assistive medical devices, any related treatments, adjust, and administer medications as and when required.
  • Commute to and from an office environment to provide care to clients within the community.
  • Act in accordance with the relevant registration body, maintain effective NMC registration and revalidation, ensuring professional portfolios are kept and updated in accordance with NMC guidelines.

  • Ability to work in the community.
  • Flexibility to work in an office environment.
