Job Summary
We are seeking an experienced Practice Nurse to join our team at Westwood Road Health Centre. The successful candidate will be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice.
Main Responsibilities
- To be responsible for the implementation of processes for the effective management of patients with long-term conditions using evidence-based practice.
- To work as a senior member of the practice multidisciplinary team, ensuring nursing services are delivered effectively to the entitled patient population.
- To be responsible for a number of clinical areas, including infection prevention and control, health promotion, chronic disease management, and well woman and well man clinics.
- To actively support the practice management team in the reviewing and delivery of clinical policy and procedure.
Requirements
- Registered Nurse with the Nursing and Midwifery Council.
- Experience in chronic disease management, including diabetes, hypertension, and respiratory conditions.
- Previous Practice Nurse experience.
- Qualified smear taker.
- Imms/Vacs trained, including childhood imms.
Desirable
- Experience using EMIS.
- Diabetes qualification.
- Asthma/COPD qualification experience.
- Travel Health.
- V300 prescriber.
