Position Overview
The Medicines Management Pharmacy Team at University Hospitals of Derby and Burton is seeking a dedicated and experienced Senior Pharmacy Technician to enhance our pharmacy services within the Cardiology specialty. This pivotal role focuses on delivering exceptional patient care through advanced medicines management practices.
This position requires a Level 4 Clinical Diploma, although we encourage applications from individuals willing to pursue this qualification. Successful candidates may initially be appointed to a band 5 Medicines Management role, with a clear pathway to progress to the band 6 Advanced Medicines Management position upon completion of the diploma.
We are in search of a Pharmacy Technician who possesses substantial post-qualification experience and has a proven track record in a clinical environment.
Key Responsibilities
The Senior Pharmacy Technician will play a crucial role in providing an enhanced clinical service focused on Medicine Management and Optimization, which includes:
- Conducting comprehensive medicine reconciliation and patient counselling.
- Performing thorough medication reviews and ensuring timely provision of discharge prescriptions.
- Guaranteeing the quality, accuracy, and safety of prescribing within the clinical setting.
- Collaborating with pharmacists and other prescribers to support patient safety through informed clinical decisions.
- Gathering detailed medication histories from various sources and reconciling them with inpatient prescriptions, addressing any discrepancies.
- Facilitating patient transfers between care settings by managing referrals via the DMS.
- Reviewing inpatient and discharge prescriptions to ensure appropriate prescribing practices and medicine availability.
- Assessing the suitability of Patients' Own Drugs for use within the Trust.
- Promoting adherence to Trust and national clinical guidelines while optimizing patient medication.
- Supporting the quality, safety, and efficiency of discharge processes, ensuring effective communication regarding medication during admissions and discharges.
- Utilizing blood results and other available data to review prescribed medications, identifying areas for necessary adjustments.
- Ensuring a safe and efficient supply of pharmaceutical products to patients, including final checking of drug products and compliance aids.
