Macmillan Assistant Practitioner/Nursing Associate

An exciting opportunity has arisen within the Manchester Macmillan Palliative Support Service for a skilled professional to join our team as an Assistant Practitioner/Nurse Associate.
About the RoleAs a key member of our multi-disciplinary team, you will contribute to the provision of a coordinated and comprehensive service for patients receiving specialist palliative care, including those residing in care homes in South Manchester.
You will work closely with our team of doctors, nurses, dieticians, occupational therapists, physiotherapists, and speech and language therapists to provide effective palliative care and support rehabilitation to patients with a diagnosis of cancer at any stage of their illness.
Main Responsibilities- Assist in the implementation, delivery, and evaluation of rehabilitation and education programs to promote independence and improve health, functional capabilities, and quality of life.
- Hold a small caseload under the supervision of the responsible clinician, supporting patients with life-limiting illnesses to maximize their health and enable them to be cared for in their preferred place.
- Collaborate with other professionals to provide comprehensive care and support to patients and their families.

Requirements- Foundation degree or equivalent qualification.
- Experience working in rehabilitation or community settings.
