This is a remarkable opportunity to enhance patient care within Primary Care Networks (PCNs). We are seeking a full-time Band 8a Senior Clinical Pharmacist to join our team, focusing on medication optimization in community settings.
As part of the NHS Long Term Plan, Hertfordshire Community NHS Trust is committed to delivering clinical pharmacy services to local PCNs. This role offers pharmacists a chance to engage in the evolving field of primary care while benefiting from the support and resources of an NHS organization.
Key Responsibilities
The Senior Clinical Pharmacist will:
- Provide expert guidance to GP practices within the PCN, enhancing the quality and efficiency of prescribing.
- Conduct clinical medication reviews and lead patient-facing clinics, collaborating with GPs, nurses, community pharmacists, and other healthcare professionals.
- Utilize your prescribing qualifications to address patient medication needs and educate healthcare teams on effective medication management.
The ideal candidate will be self-driven, possess strong organizational and communication skills, and have a solid understanding of primary care challenges related to medication management.

Qualifications and Experience
To be successful in this role, candidates should possess:
- A Master's degree in Pharmacy and registration with the GPhC.
- A postgraduate diploma in Clinical Pharmacy or equivalent.
- A non-medical prescribing qualification.
- At least five years of post-registration experience, ideally within a primary care setting.
Additionally, candidates should demonstrate a history of influencing clinical practices and managing successful medication optimization initiatives.
Skills and Competencies
Essential skills include:
- Excellent written and verbal communication abilities.
- Strong organizational skills with the capacity to manage multiple priorities.
- Advanced clinical reasoning and judgment.
- Proficiency in IT systems relevant to clinical pharmacy.
A valid driving license and access to a vehicle for business purposes are required unless exempt under the Equality Act 2010.
