About the Role

We are seeking a highly skilled and organized individual to join our team as a Discharge Facilitator. As a key member of our Capacity Team, you will play a vital role in ensuring the smooth transition of patients through our hospital, working closely with multidisciplinary teams to coordinate discharges and transfers.

Key Responsibilities
  • Coordinate the discharge or transfer of medically and therapeutically stable patients, ensuring timely and effective communication with patients, relatives, and healthcare professionals.
  • Work closely with ward managers and other internal and external agencies to ensure bed capacity is fully utilized and patients are discharged in a timely manner.
  • Provide up-to-date information on discharge or transfer arrangements for individual patients and any foreseen delays for multidisciplinary team meetings.
  • Liaise with residential and nursing homes regarding their timing of assessments for patients to return home, facilitating the process and documenting outcomes.
  • Contribute to the monitoring and facilitation of discharge for patients classed as Delayed Transfers of Care under the Delayed Discharges Act, ensuring accurate information is maintained.
Requirements
  • Education to 'Diploma' standard or equivalent, with NVQ Level III in related Health or Social Care.
  • Ability to work under pressure, meet tight deadlines, and prioritize a demanding workload.
  • Excellent communication skills, both written and verbal, with the ability to liaise effectively with professionals from other organizations.
  • Understanding of confidentiality and diversity issues, with the ability to cope with difficult and distressed patients and relatives.
  • Flexibility to work across 7 days as part of a shift rota.
