Role Overview

We are seeking a highly motivated and experienced Health Facilitation Specialist to join our dedicated team. In this role, you will play a vital part in supporting individuals with learning disabilities and promoting their well-being. You will work collaboratively with multidisciplinary teams to ensure comprehensive and holistic care.

Key Responsibilities
  • Provide expert clinical assessment and support to individuals with learning disabilities, addressing their diverse healthcare needs.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare professionals.
  • Advocate for the rights and well-being of individuals with learning disabilities, ensuring they receive equitable access to healthcare services.
  • Educate and empower individuals with learning disabilities and their families about health conditions, treatment options, and self-management strategies.
  • Collaborate effectively with multidisciplinary teams, including physicians, nurses, therapists, and social workers, to provide coordinated care.
  • Stay abreast of current best practices and advancements in learning disability healthcare.
Essential Skills and Qualifications
  • Registered Nurse with Learning Disability specialization (RNLD)
  • Demonstrated experience working within a multidisciplinary team setting.
  • Comprehensive understanding of the health inequalities faced by individuals with learning disabilities.
  • Strong clinical assessment and care planning skills.
  • Excellent communication, interpersonal, and advocacy skills.
