About the Role

We are seeking a highly skilled and dedicated Mental Health Nurse, Social Worker, or Occupational Therapist to join our Fareham Community Mental Health Team.

As a key member of our team, you will provide psychiatric interventions to a defined client group and their families within the East Hants area, ensuring the provision of a comprehensive service to those most vulnerable by virtue of mental ill health.

Main Responsibilities
  • Provide psychiatric interventions to a defined client group and their families
  • Make links with Primary Health Care teams
  • Contribute to the development and implementation of care plans
  • Work collaboratively with other healthcare professionals to ensure seamless care delivery
  • Participate in the CPA process, risk assessment, and the recovery model

  • Registered RMN/OT/Social Worker
  • Current Driving Licence
