About the Role
The Confederation, Hillingdon CIC is seeking a skilled and compassionate Healthcare Assistant to join our dynamic operations team. As a Housebound Healthcare Assistant, you will play a vital role in delivering high-quality patient care to vulnerable individuals in their homes.
Key Responsibilities
- Provide phlebotomy, ECG, and other clinical services to patients in their homes, working independently and as part of a team.
- Assess patient health needs, identify potential concerns, and escalate them to seniors as necessary.
- Screen patients for disease risk factors and early signs of illness, contributing to QOF and PCN requirements.
- Develop care plans for health-related illnesses, in consultation with patients and in line with current practice disease management protocols.
- Record clear and contemporaneous consultation notes to agreed standards, collecting data for audit purposes.
- Take responsibility for aspects of the clinical work within the team, including coordinating governance, QOF, enhanced services, CPD, and keeping clinical protocols up to date.
Requirements
- Experience of working within the NHS, with general public, and team working.
- Well-developed IT skills and an enthusiasm for new applications and software.
- Good time management and highly organized.
- Ability to use initiative, judgment, resourcefulness, common sense, and local knowledge.
- Interest in quality improvement work.
- Able to obtain and analyze complex technical information.
- Recognizes priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate.
- Able to work under pressure and to meet deadlines.
- Experience working within primary care.
- Work effectively independently and as a team member.
- Demonstrates accountability for delivering professional expertise and direct service provision.
- Able to work independently.
- Self-motivation.
- Adaptable.
- Immunisation status - Hep B / MMR.
Qualifications
- Eligible to work in the UK.
- Able to carry out duties with or without premises adaptation.
- Completed Care Certificate Training or able to demonstrate working towards completion.
