About the Role

The Confederation, Hillingdon CIC is seeking a skilled and compassionate Healthcare Assistant to join our dynamic operations team. As a Housebound Healthcare Assistant, you will play a vital role in delivering high-quality patient care to vulnerable individuals in their homes.

Key Responsibilities

  • Provide phlebotomy, ECG, and other clinical services to patients in their homes, working independently and as part of a team.
  • Assess patient health needs, identify potential concerns, and escalate them to seniors as necessary.
  • Screen patients for disease risk factors and early signs of illness, contributing to QOF and PCN requirements.
  • Develop care plans for health-related illnesses, in consultation with patients and in line with current practice disease management protocols.
  • Record clear and contemporaneous consultation notes to agreed standards, collecting data for audit purposes.
  • Take responsibility for aspects of the clinical work within the team, including coordinating governance, QOF, enhanced services, CPD, and keeping clinical protocols up to date.

Requirements

  • Experience of working within the NHS, with general public, and team working.
  • Well-developed IT skills and an enthusiasm for new applications and software.
  • Good time management and highly organized.
  • Ability to use initiative, judgment, resourcefulness, common sense, and local knowledge.
  • Interest in quality improvement work.
  • Able to obtain and analyze complex technical information.
  • Recognizes priorities when problem-solving and identifies deviations from the normal pattern and is able to refer to seniors when appropriate.
  • Able to work under pressure and to meet deadlines.
  • Experience working within primary care.
  • Work effectively independently and as a team member.
  • Demonstrates accountability for delivering professional expertise and direct service provision.
  • Able to work independently.
  • Self-motivation.
  • Adaptable.
  • Immunisation status - Hep B / MMR.

Qualifications

  • Eligible to work in the UK.
  • Able to carry out duties with or without premises adaptation.
  • Completed Care Certificate Training or able to demonstrate working towards completion.
