We are seeking a highly skilled and compassionate Substance Misuse Recovery Coordinator to join our team at Humankind - Irford House, Seacroft. As a key member of our Forward Leeds team, you will play a vital role in supporting individuals struggling with substance misuse.
Key Responsibilities- Manage a caseload of clients, creating and implementing individualized recovery and treatment plans.
- Facilitate and promote the progression of clients from initial engagement to structured treatment.
- Support clients at various stages of treatment, from assessment to aftercare, to achieve sustained recovery.
- Contribute to reducing substance-related harm to individuals and the wider community.
- Collaborate with key partners and colleagues to achieve optimal health and wellbeing outcomes for clients.
- Experience of managing a caseload and working with vulnerable individuals.
- Knowledge and/or experience of substance misuse and addiction recovery.
- Relevant qualification in Health and Social Care or a related field.
