This role will support the retired fishing community in Hull and Grimsby by providing health and wellbeing coaching services through our mobile clinic. The successful candidate will work autonomously to promote healthy lifestyles and support individuals in making positive changes to their health and wellbeing.
Main Responsibilities- Provide one-to-one and group health coaching support to individuals based on their needs and goals.
- Manage and prioritize a caseload, referring individuals to other health professionals or agencies when necessary.
- Work collaboratively with a multidisciplinary team to promote health coaching and support personalized care.
- Attend team meetings and events as required.
- Work flexibly to adapt to the needs of the service and client group.
- Participate in regular health coaching supervision and ongoing learning and development.

Person SpecificationEssential Qualifications and Knowledge
- Understanding of the biopsychosocial model of health and the social determinants of health.
- Ability to work within a biopsychosocial model, using a range of tools and techniques to enable and support individuals.
- Ability to travel across multiple sites.
- Commitment to continuous learning and development.
Essential Experience
- Demonstrable skills in supporting behavior change.
- Excellent group and one-to-one facilitation skills, including conflict resolution.
- Good understanding of physical health, including health promotion and mental health, including policy and legislation.
Desirable Qualifications and Experience
- Experience of working in a multidisciplinary team.
- Experience of working in health and social care or community development settings.
Skills and Competencies
- Proficient in Microsoft Office and web-based services.
- Ability to work with minimal supervision and act decisively when needed.
- Occasional exposure to emotional or distressing circumstances.
