About the Role

We are seeking an experienced Advanced Nurse Practitioner to join our team as a Service Lead for our Rapid Response Service in Taunton and West Somerset. As a key member of our team, you will be responsible for providing clinical leadership and operational management of the service, ensuring high-quality care and treatment for patients.

Main Responsibilities
  • Provide clinical leadership and supervision to staff within the team, ensuring a high standard of care and treatment for patients.
  • Work closely with other members of the team and other agencies to best meet the patients' health and social needs.
  • Develop and implement action plans to meet specific national and local targets, ensuring the service operates within resource limits.
  • Support the Urgent Community Response (UCR) requests, providing elements of the 9 pillars of urgent care support for patients across the county.

Requirements
  • Experience of working across organisational boundaries in a healthcare setting.
  • Experience of providing high-quality effective services within resource limits.
  • Experience of delivering services within a performance management framework.
  • Minimum 5 years Band 6 experience.
  • Knowledge of the adult health and social care agenda.
