About the Role

We are seeking a highly motivated and experienced Community Support Worker to join our team in Hartlepool. As a Community Support Worker, you will play a vital role in providing responsive and high-quality support to clients, promoting independent living and addressing their needs and aspirations.

Key Responsibilities
  • Provide flexible and person-centered support to clients, delivering practical tasks that address their needs and aspirations.
  • Draft and implement individual client risk assessments through effective communication and rapport building with the client.
  • Contribute to the review of support plans and risk assessments in a timely manner, informing all relevant parties of significant changes and alerts.
  • Work with the Support Service Coordinator to identify when clients are ready to exit the services, helping them to prepare for independence.
  • Promote independent living using positive role modeling behaviors, such as how to behave in meetings, routine appointments, and public spaces.
  • Assist in ensuring the maintenance of high-quality accommodation, effective rent collection, and efficient void and housing management.
  • Follow basic client involvement and best practice guidelines as required.
  • Comply with all internal and external audit requirements, including quality standards and accreditation and other commissioner requirements as directed.
Requirements
  • An understanding of Financial, HR, Health & Safety, and Equality & Diversity requirements.
  • Ability to use ICT appropriately.
  • Understanding of, and ability to apply, policy and procedures relating to safeguarding clients and other people at risk of abuse, Health and Safety, lone working procedures, and risk management.
  • Flexible in approach to working hours, including some weekend and evening work.
  • Emotional resilience, confidence to deal with stressful situations, lone working, and ability to use initiative.
  • Ability to achieve realistic targets and deadlines.
  • Ability to work effectively as part of a team and flexible in approach.
  • Effective interpersonal and communication skills to be able to work with a wide range of vulnerable clients.
  • Full UK driving license or access to a vehicle and able to travel across the region as required.
