As a Client Care Facilitator, you will receive specialised training and have the unique chance to work alongside multi-disciplinary teams, including clinical psychologists, occupational therapists, speech and language therapists, and rehabilitation clinicians. We are looking for individuals who can commit to a minimum of 15 hours per week, with at least 3-hour visits per client to ensure the highest quality of care.

Requirements:

Minimum 3 months of experience working with clients with brain injuries is highly desirable. Ability to provide one-on-one support in clients' homes. Commitment to promoting independence and well-being. Dedication to client safety and welfare. Strong record-keeping and documentation skills.

Key Responsibilities:

As a Client Care Facilitator, you will play a vital role in improving the lives of our clients. Your responsibilities will include:
