Position Overview

We are seeking a dedicated part-time Clinical Nurse to become a vital member of our supportive and caring team. The successful candidate will be responsible for providing comprehensive assessment, treatment, screening, health education, and guidance to our patients.

Working Hours

The role requires a commitment of 30 hours per week, scheduled from Monday to Friday. Specific hours can be discussed during the interview process.

Key Responsibilities

The core duties of the Clinical Nurse include:

  • Management of chronic conditions such as COPD, Asthma, Hypertension, Cardiovascular Disease, and Chronic Kidney Disease.
  • Overseeing Diabetes care and management.
  • Administering Childhood Vaccinations.
  • Conducting Health Checks.
  • Performing Cervical Cytology.
  • Promoting Health and Wellness initiatives.

About Our Practice

Our team comprises Advanced Nurse Practitioners, Practice Nurses, and Health Care Assistants, all supported by the Practice Manager and GP Partners, including a designated GP Nursing Lead. Job Duties

The following are essential responsibilities of the Clinical Nurse:

  • Support the Infection Prevention Control (IPC) lead by adhering to IPC policies and assisting with audits and training.
  • Contribute to the development and implementation of effective care management programs for frail and housebound patients.
  • Assess patient needs to ensure appropriate healthcare delivery, utilizing evidence-based practices.
  • Implement and evaluate individual treatment plans for chronic disease patients.
  • Conduct home visits as necessary.
  • Identify and manage patients at risk of developing long-term conditions.
  • Provide routine nursing care in accordance with clinical guidelines.
  • Diagnose and manage both acute and chronic health conditions.
  • Prescribe medications within the scope of practice.
  • Review patient medications to enhance adherence.
  • Coordinate travel medicine services.
  • Request and process pathology services as needed.
  • Maintain accurate clinical records in compliance with relevant legislation.
  • Support health needs of women through cervical cytology and promote attendance.
  • Assist GPs with minor surgical procedures when required.
  • Participate in clinical emergencies and provide support to the team.
  • Implement vaccination programs for all age groups.
  • Collaborate with external services to ensure comprehensive patient support.

Qualifications and Experience

Essential:

  • Registered Nurse with the Nursing and Midwifery Council.
  • Experience in chronic disease management and working autonomously.
  • Knowledge of infection prevention and control measures.

Desirable:

  • Experience in a primary care setting.
  • Experience as a practice nurse or community nurse.
  • Experience managing elderly and housebound patients.
