Key Responsibilities- Hold a clinical caseload and coordinate care planning for families with high social care needs and associated emotional distress.
- Participate in the duty rota and provide a more immediate response to families or referrers as needed.
- Offer advice and guidance on risk and safeguarding to the team, completing assessments of risk and referring to social care teams where necessary.
- Liaise with relevant agencies to deliver comprehensive packages of care for families with social system needs.
- Work with colleagues to ensure delivery of high-quality care for families with complex needs.
