Position Overview

An exceptional opportunity has emerged for a dynamic role as a Prostate Care Pathway Facilitator, aimed at delivering extensive administrative assistance in collaboration with the Urology Specialist Team. This position is pivotal in advancing the care journey for all individuals suspected of having Prostate Cancer.

The ability to communicate in Welsh is preferred; applications from both Welsh and English speakers are encouraged.

Key Responsibilities

• Engage with both clinical and non-clinical members of the healthcare team to enhance patient care, coordinating appointments or referrals to other services within or outside the organization.

• Ensure that the patient's progression through the care pathway is managed proactively, addressing any issues related to waiting times and treatment schedules with the appropriate personnel or clinical services.

• Identify patients requiring clinical decisions and utilize sound judgment to ensure timely actions are taken concerning individual diagnostics.
