Job Summary:
The Extra Care Scheme Manager will play a crucial role in overseeing and ensuring the delivery of high-quality care services to individuals with varying needs. This position is based on-site at our Extra Care facility and involves working closely with a dedicated team of healthcare professionals to maintain and promote the well-being and independence of our service users.
Key Responsibilities:
- To input all initial visit paperwork onto the care management system and use this to inform the Service Users Care Plan, focusing on person-centred care.
- To be responsible for giving feedback regarding any issues or changes regarding Service Users care to social workers, family members, care staff, and other professionals involved.
- To ensure risk assessments relating to individual Service Users are updated as necessary following any hospital discharges or changes to their care.
- To compile rotas for care workers.
- To assign care workers to customers, whilst working within our care standards.
- To understand, organise, and record the availability of all new care workers.
- To work closely and communicate with other members of the team, professional bodies, and family members to ensure the highest quality of care.
- To accept, allocate, and process referrals for new care packages.
- To accept and process changes and amendments to existing care packages.
- To manage customer visits and arrange reviews and quality checks.
- To monitor quality through care records and weekly KPI reports.
Requirements:
The suitable applicant will have experience of working within a care environment and have carried out tasks similar to those listed above.
Essential Qualities:
The successful candidate will possess excellent communication and organisational skills, with the ability to work effectively in a fast-paced environment.
