Key Responsibilities
As the Lead Social Care Practitioner, you will play a vital role in supporting our elderly residents and their families who face intricate social challenges and vulnerabilities due to various factors such as disability or diminished capacity. Your mission will be to empower them to achieve greater independence.
You will concentrate on the most intricate and demanding resident cases, ensuring the safety and well-being of both residents and their families. Collaboration is key; you will share insights and best practices while providing expert guidance as part of a multidisciplinary team, leveraging your expertise as a Social Care Practitioner.

Role Responsibilities
The Lead Social Care Practitioner will be tasked with delivering specialized advice and support to nursing homes and their multidisciplinary teams, facilitating effective resident discharges and preventing delays in care transfers as part of comprehensive care management across the Trust.
You will be responsible for conducting Care Act assessments, evaluating social care funding eligibility, and ensuring all necessary documentation is completed. Additionally, you will support colleagues by reviewing their paperwork and collaborating with relevant agencies.
Qualifications and Skills
Essential Qualifications
- Recognized Social Work Qualification (Degree in Social Work, DipSW, CQSW, CSS, or equivalent)
- Registration with Social Work England as an Approved Mental Health Professional
Knowledge and Experience
- Experience in multidisciplinary social care settings, particularly with individuals facing mental health challenges.
- Proficiency in conducting assessments in line with the Mental Health Act and the Mental Capacity Act.
- Competence in IT for effective data management.
Personal Attributes
- Ability to work independently and manage pressure effectively.
- Capacity to respond calmly to unexpected challenges, seeking solutions and adjusting plans as necessary.
- Awareness of personal stressors and proactive in seeking support to maintain well-being.
