About the Role

We are seeking a highly skilled and experienced Mental Health Specialist Practitioner to join our Early Intervention in Psychosis Service team at Somerset NHS Foundation Trust.

Key Responsibilities
  • Provide assessments, care coordination, outreach, and support to individuals and their families affected by first episode psychosis.
  • Work autonomously with a strong team of healthcare professionals to deliver individual therapy, family interventions, and education.
  • Develop and implement care plans in conjunction with clients and carers, ensuring a comprehensive and person-centered approach.
  • Collaborate with other agencies and services to provide a seamless and effective care pathway.
  • Contribute to the development and implementation of service policies and procedures.
Requirements
  • Registered Mental Health Nurse, Social Worker, or Occupational Therapist with current registration and post-qualifying experience in an acute setting.
  • Significant experience in multi-disciplinary community mental health services, with a strong understanding of Early Interventions principles and research.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and colleagues.
  • Ability to work autonomously and independently, with a strong focus on patient-centered care.
  • Experience of managing risk, assessing, and developing care plans, with a strong understanding of mental health legislation and policies.
Desirable Qualifications and Experience
  • Postgraduate degree or diploma in a relevant field, such as family interventions in psychosis or cognitive behavioral therapy for psychosis.
  • Experience of working with people with mental health difficulties, including first episode psychosis.
  • Knowledge of resources and working practices across health and social care, with a strong understanding of the Somerset NHS Foundation Trust's services and policies.
