About the Role

We are seeking a skilled and compassionate Reablement Assistant to join our team at North Lincolnshire Council. As a key member of our Community Rehabilitation service, you will play a vital role in supporting individuals to regain their independence and confidence in their own homes.

Key Responsibilities:

  • Providing direct support with daily living tasks, including personal care and medication administration
  • Assisting with moving and handling to promote mobility and independence
  • Facilitating social inclusion and emotional wellbeing through engaging activities
  • Working collaboratively with the transitions team to ensure smooth transitions between hospital and rehabilitation settings

Requirements:

  • Previous qualification in care (desirable but not essential)
