Position Overview

The Adam Practice is excited to announce a rewarding opportunity for a dedicated and skilled Pulmonary Care Nurse to become a vital part of our healthcare team.

Role Objective

The primary focus of this position is to deliver exceptional, patient-centered care to individuals with diagnosed or suspected pulmonary conditions, whether in their residences or within a practice-based clinic. This role emphasizes a comprehensive, integrated, and evidence-driven approach to managing respiratory health, which includes conducting annual assessments, follow-up evaluations post-discharge, and interim assessments after acute exacerbations to minimize readmission risks.

Work Schedule

This position is part-time, comprising 12 hours weekly, with 4 hours allocated for clinic duties on Friday mornings and the remaining 8 hours to be scheduled on either Wednesday, Thursday, or Friday afternoons, as discussed during the interview process.

Key Responsibilities

  • Actively participate as a member of the Pulmonary Care Team to facilitate the delivery of respiratory diagnostic services, including clinical evaluations, spirometry, and FeNO testing, while supporting the diagnosis of respiratory illnesses within the local patient population.
  • Collaborate closely with the broader Enhanced Care Team to assess patients in their homes requiring annual respiratory evaluations or referrals from other healthcare teams.
  • Conduct thorough and holistic respiratory assessments, utilizing specialized skills to evaluate both the physical and psychosocial needs of patients with respiratory conditions.
  • Develop personalized, evidence-based care plans in partnership with patients, promoting long-term management of their conditions and encouraging adherence to treatment protocols.
  • Review respiratory medications for efficacy and appropriateness, adhering to established clinical guidelines and protocols.
  • Provide cognitive assessments and identify mental health needs, coordinating with multidisciplinary colleagues as necessary.
  • Facilitate community-based care by working alongside other health and social care providers to meet individual patient needs.
  • Identify and address social isolation, connecting patients with resources to encourage active community engagement.
  • Utilize effective communication skills to build strong relationships with patients, families, and caregivers.
  • Recognize and respond to health deterioration promptly to mitigate risks of rapid decline or unnecessary hospital admissions.
  • Educate patients and caregivers on recognizing early signs of health deterioration to enable swift management of potential complications.
  • Support early hospital discharge for case-managed patients by coordinating care and services within the community.
  • Identify patients with complex health needs and refer them for comprehensive interdisciplinary assessments.

Qualifications

Essential:

  • Registered Nurse with a postgraduate qualification in respiratory disease management (e.g., COPD or asthma).

Desirable:

  • Mentorship certification.
  • Accredited training in spirometry and/or FeNO testing.

Experience

Essential:

  • Experience in managing patients with respiratory conditions in primary or community care settings.
  • Ability to work independently and support peers and junior colleagues.

Desirable:

  • Experience in service improvement initiatives.
