Position Overview

The West Locality is seeking to appoint a Band 5 Rehabilitation Therapist to join the Intermediate Care Therapy Team. This role involves collaboration with the Urgent Community Response Therapy Team, addressing all facets of the Intermediate Care Team's responsibilities.

The Therapy Team is dedicated to providing high-quality, evidence-based support to patients in both rehabilitation facilities and their residences, aiming to promote early supported discharge and prevent unnecessary hospital admissions.

The ideal candidate will demonstrate a strong commitment to teamwork and the delivery of comprehensive, patient-centered care. The role requires conducting therapy evaluations for patients in rehabilitation settings and within the urgent community response framework, assessing individuals in their homes. Community visits may be scheduled or spontaneous, depending on patient needs.

Clinical guidance will be offered by a Band 7 Clinical Lead Physiotherapist and a Band 6 Occupational Therapist. We provide a supportive environment with opportunities for continuous professional growth.

Key Responsibilities

The primary duties include:

1. Conducting assessments, prescribing, and implementing therapeutic interventions for patients with chronic conditions, elderly individuals, those requiring palliative care, and rehabilitation support, with the goal of enhancing quality of life and independence.

2. Collaborating within the integrated team to facilitate timely discharges from healthcare facilities.

3. Working alongside the integrated team to avert unnecessary hospital admissions.

4. Providing in-reach services to inpatient units and step-up care facilities as necessary.

5. Coordinating with healthcare professionals and various agencies to ensure a cohesive, integrated service for our clients.

Clinical Duties:

The post holder will:

Perform thorough occupational therapy assessments of patients, utilizing investigative, analytical, and clinical reasoning skills to develop tailored treatment plans that may involve diverse therapeutic options. Create, monitor, and adjust individualized occupational therapy treatment plans in collaboration with the integrated team, other agencies, and the patient, employing various treatment techniques and equipment to enhance functional capabilities. Stay informed about Assistive Technology and make referrals as necessary. Evaluate patients' holistic needs, effectively communicating complex and sensitive information regarding their assessment, diagnosis, prognosis, and treatment plans. Adapt explanations to accommodate barriers to understanding, such as hearing impairments or cognitive challenges, ensuring informed consent and cooperation. Utilize tact and persuasive skills to encourage patient engagement in their treatment plans. Collaborate with GPs, Social Services, inpatient teams, other allied health professionals, and the voluntary sector to ensure comprehensive care coordination. Participate in multi-disciplinary and multi-agency meetings as appropriate. Manage and prioritize workload effectively to meet service targets and needs. Ensure accurate documentation for each patient visit and complete daily inputting in the relevant systems. Demonstrate dexterity and coordination when using specialized equipment and tools, as well as manual and mobilization skills. Assist in the requisitioning of supplies and equipment through electronic ordering systems. Obtain informed consent prior to initiating any interventions. Be prepared to handle bodily fluids and other potentially infectious materials, adhering to universal precautions and infection control policies.
