Job Summary

We are looking for a skilled and motivated Health Check Professional to work with us in the community, providing health checks and promoting healthy lifestyles to our patients. As a Health Check Professional, you will be responsible for conducting health checks, providing patient education and health promotion, and working to attain service KPIs and targets.

Key Responsibilities

  • Conduct health checks in the community, face-to-face with patients
  • Provide patient education and health promotion
  • Work to attain service KPIs and targets
  • Ensure all patient contact is recorded in a timely manner on the IT system
  • Acknowledge and accommodate barriers to communication and understanding
  • Flexibility in attending organised educational updates and eagerness to expand on current knowledge base
  • Inform the Clinical Manager of any untoward incidents

Requirements

  • Experience of working in GP Practice or community
  • Able to undertake clinical observations
  • Have a knowledge of NHS Health checks
  • Good standard of general education
  • NVQ Level 3 or equivalent
  • Ability to work as a team member
  • Able to exercise initiative in organisation and prioritising
  • Good communication skills both written and oral
  • Good Interpersonal skills
  • Basic IT skills
