About the Role

We are seeking a highly skilled Mental Health Practitioner to join our team in Central Lancashire. As a key member of our multidisciplinary team, you will play a crucial role in delivering NHS community mental health services to children and young people.

Key Responsibilities
  • Participate in multi-disciplinary assessments of patients' health conditions.
  • Formulate and implement care plans.
  • Provide treatment as necessary.
  • Use evidence-based practice and specialist skills to assess, plan, implement, and evaluate interventions.
  • Collaborate with other healthcare professionals to ensure optimal patient care.
  • Maintain accurate and up-to-date records in line with professional standards.
  • Stay current with the latest mental health advancements and best practices.
Requirements and Qualifications
  • Current and valid registration with the NMC.
  • Proven experience within a community recovery role.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
