Job Summary:
Direct Medics is seeking a highly skilled and dedicated SHO to join our team in providing acute care at home services across the Southern Trust area. As a key member of our healthcare team, you will be responsible for delivering high-quality patient care in a home-based setting.
Key Responsibilities:
- Provide comprehensive acute care services to patients in their homes, including assessment, diagnosis, and treatment.
- Work collaboratively with multidisciplinary teams to ensure seamless care coordination and patient outcomes.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Conduct regular patient assessments and evaluations to monitor progress and adjust care plans as needed.
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making and optimal care.
Requirements:
- SHO qualification with relevant experience in acute care.
