About the Role

We are seeking a highly motivated and compassionate Support Coordinator to join our team at Home Group Limited. As a Support Coordinator, you will play a vital role in providing person-centered support to individuals with complex mental health needs, enabling them to develop the skills and confidence to live more independently in the community.

Key Responsibilities
  • Creating and implementing person-centered support plans with customers, working closely with clinicians to develop Risk Management Plans.
  • Supporting customers with independent living skills, such as budgeting, maximizing income, cooking, and engaging with external services.
  • Promoting Home Group values within the team and working collaboratively with local authorities, NHS mental health teams, safeguarding teams, and housing option teams.
Requirements
  • Passion for supporting customers to live their best life, working together with an eye for detail.
  • Experience of coordinating and signposting to internal and external services.
  • Ability to work independently, remain calm under pressure, and have a resilient approach.
  • Flexibility to adapt to changing circumstances and priorities.
