Belmont Recruitment is seeking a qualified Community Care Officer to join a dedicated team focused on supporting adults with diverse needs. This position is offered on a temporary basis and involves full-time hours of 37 per week, with a hybrid working model available after initial training.

The Adult Duty Team is committed to assisting individuals aged 18 and over, ensuring they receive comprehensive, person-centered assessments that emphasize strengths and promote independence.

Our approach involves close collaboration with partner organizations to enhance the safety and autonomy of adults in the community, prioritizing early intervention and preventative measures.


Key Responsibilities:

  • Conduct Care Act assessments, utilizing both face-to-face and telephone methods, while considering mental capacity and individual risk factors.
  • Deliver tailored services to protect vulnerable adults and their families, based on assessed needs.
  • Perform various levels of assessments in accordance with departmental eligibility criteria and national performance standards, ensuring timely completion.
  • Develop and implement culturally sensitive care plans that reflect flexibility and innovative resource utilization, taking on a care coordination role.
  • Manage a caseload effectively, including risk management, monitoring, coordinating, and reviewing care plans and service delivery.
  • Work autonomously under the guidance of the Team Leader, meeting deadlines and adapting to dynamic circumstances and competing priorities.
  • Support team members by providing guidance, mentorship, and assistance as needed, which may include escorting service users and conducting home visits.
  • Maintain professional case records and reports that are structured, accurate, timely, and evidence-based, adhering to departmental standards.

Essential Qualifications:

  • Proven experience in a social care environment working with adults.
  • Familiarity with challenges faced by adults, including learning disabilities, mental health issues, substance misuse, and physical health concerns.
  • Understanding of safeguarding practices and the ability to identify individuals at risk, demonstrating flexibility and resilience in changing situations.
  • Competence in conducting adult-focused needs assessments with a person-centered approach.
  • Experience in creating and executing personalized care plans that showcase adaptability and innovative resource management.
  • Knowledge of costing and commissioning care packages.
