Health Care Support Worker

We are seeking a highly motivated and experienced Health Care Support Worker to join our Community Learning Disability Team in Knowsley. As a key member of our team, you will play a vital role in supporting individuals with learning disabilities and complex needs to live independently and participate fully in their communities.
Key Responsibilities- Support the delivery of specialist health support to promote independence, community inclusion, and positive reputation of individuals with learning disabilities.
- Assist in the implementation of care plans and programmes, including reporting outcomes and contributing to the recording of plans and programmes.
- Act as a nominated worker for individual clients, providing therapeutic activity and support in their homes or in the community.
- Facilitate and support the development of appropriate personal care, self-help, communication, and social skills acquisition.
- Assist and promote independence with activities of daily living, including participation in social, vocational, recreational, and leisure activities.
- Participate in the implementation of specific programmes of care or therapy to develop and enhance socially appropriate adaptive behaviours under the supervision of qualified staff.
- Develop effective interpersonal relationships with colleagues and service users, and contribute to a positive and inclusive team environment.
- NVQ 3 or equivalent level of experience within a care setting.
- Experience of working with service users with learning disabilities and complex needs.
- Knowledge of the Recovery model and an understanding of the organisation of nursing care.
