Key Responsibilities- Deliver person-centred care to patients with life-limiting illnesses, focusing on their physical, emotional, and social needs.
- Work collaboratively with the multidisciplinary team to develop and implement individualized care plans.
- Provide emotional support and companionship to patients and their families, promoting dignity and respect.
- Assist with daily living activities, such as bathing, dressing, and medication management.
- Monitor patients' symptoms and report any changes to the healthcare team.
- Maintain accurate and detailed records of patient care and progress.
- Relevant experience in healthcare, preferably in palliative care or end-of-life care.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
- Ability to work independently and as part of a team, with a flexible approach to working hours.
- Basic Food Hygiene Certificate and Functional Skills at Level 2 in numeracy and literacy.
- Valid UK driving license and access to a car for business use.
