About the Role

We are seeking a skilled Clinical Practitioner to join our Central Norwich Home Visiting Service, a new initiative created by Castle Partnership to provide high-quality healthcare to patients in their homes.

Key Responsibilities

As a Clinical Practitioner, you will work closely with our multi-disciplinary team to assess, diagnose, and treat patients in their homes or care facilities. Your duties will include:

  • Conducting home visits to assess patients with acute presentations of illness
  • Diagnosing and treating patients in their own homes
  • Liaising with our practice team to ensure referrals and signposting to relevant agencies
  • Attending Clinical Governance and MDT meetings within the practices
  • Supporting with Long Term Condition and Chronic Disease Management
  • Identifying areas of Health Inequalities and communicating this to the practices to enable service improvement
  • Liaising with GPs regarding patients who may need to be moved to a Palliative Pathway
  • Working as part of a team, both the practice teams and the Central Norwich Home Visiting Team

Requirements

To be successful in this role, you will need:

  • Qualified Paramedic Practitioner or Nurse Practitioner
  • General Practice/Community experience
  • Full UK Driving License
  • Ability to work as a team, or alone
