Job Title: Intensive and Enablement Service Support Worker

**Job Summary:**

We are seeking a dedicated and compassionate Support Worker to join our Intensive and Enablement Service team. As a Support Worker, you will play a vital role in supporting individuals with Learning Disabilities, Autism, and Mental Health needs to develop their daily living skills and improve their quality of life.

**Key Responsibilities:**

  • Provide person-centered support to individuals in their homes or in the community
  • Develop and implement personalized support plans to promote independence and problem-solving skills
  • Work collaboratively with individuals, families, and other professionals to ensure seamless support
  • Participate in regular training and development opportunities to enhance your skills and knowledge
