We are seeking a highly skilled and dedicated Band 5 Community Nurse to join our team at Mid Cheshire Hospitals NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes and communities.
Key Responsibilities:- Assess and manage the health needs of patients in their own homes and communities.
- Develop, implement, and evaluate programs of care to meet the needs of patients with long-term conditions and complex nursing needs.
- Work collaboratively with other healthcare professionals and agencies to ensure effective service delivery.
- Provide skilled nursing care, including wound care, leg ulcer assessment and management, and administration of medication via syringe pump.
- Recognize, interpret, and analyze clinical data related to individual client needs, including client observations.
- Teach and supervise nursing care that can be safely delegated to patients and carers.
- Participate in the delivery of care involving complex needs and supporting carers and relatives in distressing circumstances.
- UK driving license and use of car.
- Knowledge of Long Term Condition Management.
- Registered General Nurse/Registration Imminent.
- Community Experience.
- A listening, learning, and leading approach.
- Experience of multi-agency working.
Part-time.
Contract:Permanent.
