Role Overview

As a Qualified Practitioner, you will manage a designated caseload that includes complex cases. You will adopt a Person-Centered approach, conducting strength-based Adult Needs Assessments and Reviews that emphasize the capabilities of individuals rather than their limitations. Collaborating with service users and their families, your goal will be to enhance their independence.

Our mission is to empower individuals to achieve their personal goals and aspirations, whether that involves securing employment, accessing community resources, or transitioning to independent living. We assist individuals in overcoming barriers and challenges, enabling them to realize their full potential.

Travel to various appointments is a requirement of this role, which may include out-of-county travel. It is essential that you can travel efficiently and within a reasonable timeframe to meet service demands, including responding to unplanned visits to non-local appointments. Access to personal transportation is crucial.

Claims for work-related travel expenses will be reimbursed.

Our standard office hours are Monday to Thursday from 08:45 to 17:15 and Friday from 08:45 to 16:45. We currently offer a blend of remote and office-based work, with a requirement to be present in the office as needed to fulfill service obligations.

Candidate Profile

We value our teams and the services we provide; therefore, it is vital to recruit individuals who can contribute positively to our mission and support our service users effectively.

The ideal candidate will possess either a Social Work or Nursing qualification and must maintain an active registration with the relevant professional body. Note: The nursing qualification does not need to be specific to Learning Disabilities.

This position is not suitable for newly qualified candidates who have not yet completed their ASYE.

Qualified candidates should demonstrate the following skills and attributes:

  • A proactive attitude, self-motivation, and enthusiasm.
  • Respect for individuals in their homes, ensuring dignity is upheld.
  • Strong communication skills, with the ability to understand both verbal and non-verbal cues, and the confidence to exhibit professional curiosity.
  • Creative problem-solving abilities.
  • Ability to perform well under pressure or in emergency situations.
