Job Overview

The post holder will be responsible for delivering specialist nursing care as part of a multidisciplinary clinical team. The focus will be on the open access follow up care of patients treated for breast cancer, as well as supporting patients with breast cancer at all stages of their pathway.

Main Duties of the Job
  1. Develop, implement, and monitor the open access follow up of patients treated for breast cancer in conjunction with the advanced nurse practitioner and other members of the multidisciplinary team.
  2. Provide clinical leadership and act as a key worker for people diagnosed with breast cancer and being followed up on the open access pathway, ensuring each person receives a high standard of personalized care consistent with the Macmillan Right by You Principles.
  3. Provide supportive conversations at key times in the patient's journey, particularly at the time of diagnosis and during open access follow up as required.
  4. Conduct holistic needs assessments at the time of diagnosis, at the end of treatment, and at any time during open access follow up when the patient's condition or situation requires it.
  5. Develop personalized care plans at the points in the patient's pathway previously mentioned.
  6. Provide treatment summaries and health and wellbeing interventions to all patients.
  7. Provide appropriate information working closely with the Macmillan team.

Essential Criteria

  1. Current NMC Registration
  2. Breast cancer training
  3. Ability to work independently and as a team member
  4. Ability to influence current practice and influence change
  5. Ability to communicate via a range of mediums to a wide range of audiences
  6. Relevant post-registration experience in breast cancer
  7. Proven leadership and management skills
  8. General breast cancer knowledge
  9. Understanding and demonstration of the Trust Values
