Position Overview
We are excited to present a unique opportunity for a dedicated individual to step into the role of an Advanced Pharmacist Practitioner within the Emergency Department. This position is ideal for those who are eager to develop their clinical expertise and contribute to the enhancement of patient care.
Role Responsibilities
The successful candidate will be responsible for:
- Delivering a clinical practitioner service to patients in the Emergency Department.
- Providing expert advice on medication management at both ward and consultant levels.
- Conducting comprehensive medication assessments and participating in multidisciplinary team evaluations.
- Engaging in discharge planning and patient education, while facilitating referrals through established services.
- Identifying opportunities for service improvement in collaboration with the Lead Pharmacist.
- Mentoring junior pharmacists and fostering a supportive learning environment.
- Working flexible hours, including weekends and holidays, as part of a rotational schedule.
Candidate Profile
We are looking for individuals who possess:
- A proactive approach to influencing medication practices within a healthcare setting.
- A postgraduate clinical diploma.
- An independent prescribing qualification or progress towards it.
- A commitment to teamwork and collaboration in all aspects of work.
- Experience in service development and the implementation of improvements.
