Care and Support Professional

We are seeking a dedicated and compassionate Care and Support Worker to join our team at Holt House, a residential care home in Prestwich. As a key member of our care team, you will play a vital role in providing high-quality care and support to our residents, ensuring they enjoy the highest possible quality of life.
Key Responsibilities- Contribute to person-centred assessment planning, implementation, and review, working closely with residents, families, and other healthcare professionals.
- Deliver excellent person-centred care, recognising the unique needs and preferences of each resident.
- Act as a key worker for allocated residents, providing emotional support and guidance to help them maintain their independence.
- Support residents with their daily living tasks, promoting their physical and emotional well-being.
- Promote equality, diversity, and inclusion, ensuring that all residents feel valued and respected.
- Share in person-centred assessment planning, implementation, and review, working collaboratively with the care team.
- Recognise and respond to the social, emotional, and spiritual needs of residents, delivering activities that promote their well-being.
- Remain alert and attentive to the needs of residents throughout the shift, ensuring their needs are met at all times.
- Enable interaction and communication, adapting to the unique needs of residents with dementia.
- Ensure that your actions support the care, protection, and well-being of residents.
- Help address the physical comfort needs of residents, promoting their dignity and respect.
- Support residents with their personal care needs, including managing continence and mobility.
- Provide support for residents to monitor their own health care needs, including supporting the use of medication and administering if trained and qualified.
- Support residents in journeys to and from the home, ensuring their safety and well-being.
- Engage with residents to participate in recreational and development activities, promoting their social and emotional well-being.
- Provide food and drink to residents, assisting them with eating and drinking if necessary.
- Respond to residents' refreshment requests throughout the shift, ensuring their needs are met in accordance with their individual care plans.
- Support residents who are distressed, remaining respectful and caring at all times.
- Support residents at the end of their life, providing compassionate care and support.
- Observe, monitor, and record the condition of residents, reporting as required, and communicating with families and other healthcare professionals.
- Complete records for residents, contributing to care plans and ensuring accurate and timely documentation.
- Support the health and safety of yourself and residents, complying with relevant legislation and procedures.
- Attend and participate in staff meetings, contributing to the development of the care team.
- Participate in supervision and appraisal, reflecting on your practice and identifying areas for improvement.
- Share in a flexible rolling rota, ensuring the smooth operation of the care home.
- Develop your knowledge and practice through training, ensuring you remain up-to-date with best practice.
- Follow the guidance set out in the Code of Conduct for Healthcare Support Workers and Adult Social Care Workers in England.
- Work positively as a member of the care team, promoting effective and harmonious relationships with all staff members.
- The post holder must comply with the Salvation Army's dress code.
- Undertake other duties as deemed necessary to ensure the smooth running of operations and fulfill The Salvation Army's mission.
- Experience in caring for someone in your personal life.
- Experience working in Adult Social Care.
- L2 Health and Social Care desirable but not essential.
Full-time shifts are 35 hours paid per week, with a competitive salary of £11.25 per hour.
Early shift: 07:30am - 2:30pm
Late shift: 2:30pm - 9:30pm
Schedule: Monday to Friday, with some weekend work.
