An exciting opportunity has arisen for a registered nurse to contribute to the Learning Disability Liaison Nurse Team in Oxford. Main Duties
  • Ensure the needs of individuals with learning disabilities are met when they require care from the Trust's clinical services, facilitating access to various wards and departments and promoting effective communication between individuals with learning disabilities, their families/carers, and Trust clinical teams.
  • Provide exceptional patient care underpinned by evidence-based practice and OUH values.
  • Work proactively and collaboratively with carers, family, and support staff to ensure care is coordinated across agencies.
  • Provide specialist nursing care for the team's caseload of patients.
  • Communicate effectively with patients/relatives/carers and all members of the multidisciplinary team.
  • Contribute to the development of services.
  • Demonstrate, develop, and maintain specialist knowledge of learning disability nursing practice to act as a clinical expert and professional resource.
  • Undertake responsibilities associated with being a mentor and preceptor, reflecting beliefs about professional nursing practice and patient care.
  • Conduct oneself in a professional manner and be a role model to others at all times, commensurate with The Code (NMC).
