About the Role

We are seeking an experienced Clinical Specialist Nurse to join our team at Camden and Islington NHS Foundation Trust. As a Clinical Specialist Nurse, you will work alongside a multidisciplinary team to provide holistic care to individuals with complex mental health needs.

Main Responsibilities
  • Work as part of a multidisciplinary team to deliver comprehensive care to individuals with complex mental health needs.
  • Collaborate with service users to complete thorough assessments and care plans.
  • Hold a defined caseload of service users and manage their care delivery.
  • Complete thorough risk assessments and formulations for service users on your caseload.
  • Raise safeguarding concerns and act as an enquiry officer for your cases and any delegated cases.
  • Ensure all incidents, near misses, or actual events are reported to your manager and reported through our incident reporting system.
  • Take on duty person responsibilities as assigned and attend and occasionally chair daily morning huddles and weekly clinical meetings.
  • Maintain sufficient contacts with service users face-to-face, telephone, and other media, completing diary appointments and outcoming them promptly.
  • Make timely and accurate case note entries.
  • Communicate effectively with the multidisciplinary team both in and outside meetings and liaise with other statutory services and partnership agencies.
  • Undertake care act assessments, mental capacity assessments, and play an active role in best interests decisions.
  • Complete support plans and arrange to attend funding panel in a timely manner for service users requiring packages of care and review the same support plan at set dates.
  • Ensure all eligible service users have s117 care plans agreed and completed at point of discharge and referred to funding panel. Complete s117 annual reviews following Dialog + and medical reviews.
  • Take the lead in assessing and addressing service users' physical health needs and get involved in supporting their access to general healthcare and specialisms.
  • Ensure an annual physical health tool is completed for all your caseload.
  • Work with partner agencies to meet service users' needs.
  • Undertake MHAs if qualified as an AMHP.
  • Manage medications for the whole team alongside other nurses and administer depot injections safely, at the centre, and in service users' homes while monitoring their compliance, efficacy, and side effects. Ensure documentation accurately both on the chart and on our electronic patient record system.
  • Ensure stock and individual medications are ordered and stored in a timely manner and correctly.
  • Ensure there is consent or SOAD certificate on all service user medication charts.
  • Ensure s132 rights for any service users on CTO are presented to them whenever CTO is renewed.
  • Complete social circumstances reports for detained in-patients and community patients on CTO who are on your caseload or when asked to cover.
  • Liaise with inpatient ward colleagues and attend Ward Rounds for in-patients and assist in arranging safe and early discharge, ensuring 72 hours follow-up is completed.
  • Attend Tribunal or Associate Hospital Managers hearing for your service users or when asked to cover for colleagues.
  • Take the lead in infection prevention and control.
  • Support student nurses and other students by acting as a supervisor and assessor.
  • Supervise Assistant Practitioners when assigned.
  • May on occasions be asked to deputise on behalf of team manager.
  • Take on Lead or Champion Roles for specific tasks or areas of the service.
  • Attend monthly supervisions and annual appraisal.
  • Complete all statutory and mandatory trainings in a timely manner.
  • Identify learning needs and discuss with your supervisor/line manager.
  • Offer orientation, induction, and preceptorship to new staff.
  • You will be an agile worker able to prioritise and reprioritise to manage demand and flow.
  • Identify and inform your manager/senior practitioner tasks to be completed if working from home.
  • To be proactive and take initiative to improve service delivery, using quality improvement methodology.
  • Use evidence-based practice to deliver care (NICE guidelines, Royal College of Psychiatry).
  • A good understanding of trauma-informed care and its implementation in care and support.
  • Keep up to date with local and national changes in care delivery and understand how they will impact care delivery.
Person Specification

Essential:

  • 2 years post-registration experience.
  • Evidence of further post-registration relevant training/ongoing academic study relevant to area of practice.
  • Mentorship or recognised teaching and assessing certificate or willingness to undertake.

Desirable:

  • Other advanced training AMHP/NMP/CBT in Psychosis.

Knowledge and Experience:

Essential:

  • Demonstrable experience in a senior specialist capacity caring for people with psychosis in a community care or inpatient setting.
  • Demonstrate a sound knowledge and understanding of the needs and difficulties of people with complex mental health and/or substance misuse problems and the impact on families.
  • Demonstrate a clear understanding of the Mental Health legislation, its application, and use in practice.
  • Demonstrate an ability to liaise with other agencies, carers, and the service user's wider support network.

Desirable:

  • An awareness of research and evidence of its application in the workplace.
  • Evidence of collaboration in quality improvement initiatives.

Skills/Abilities:

Essential:

  • Excellent verbal and written communication skills.
  • Able to work autonomously and to deputise for senior staff in a variety of roles when required to do so.
  • Competent using computers and keyboards for entering personally generated data, generating and responding to emails, conducting web searches, storing data, and retrieving data, producing reports, etc.

Desirable:

  • Demonstrable experience of adaptability to change.
