About the Role:

We are seeking a compassionate and dedicated Support Worker to join our team at We Change Lives, a charity dedicated to empowering individuals with learning disabilities, physical disabilities, and mental health issues. As a Support Worker, you will play a vital role in providing person-centered support to our service users, helping them to live fulfilling and independent lives.

Key Responsibilities:

  • Providing personal care and support to individuals with learning disabilities, including assistance with daily living tasks and promoting independence.
  • Developing and maintaining positive relationships with service users, their families, and other professionals.
  • Participating in person-centered planning and contributing to the development of individualized support plans.
  • Supporting service users to access community resources and activities, promoting social inclusion and participation.
  • Working collaboratively with the multidisciplinary team to ensure high-quality care and support.
