We are seeking a highly motivated and innovative Community Health Nurse to join our team at Your Healthcare CIC. As a Community Health Nurse, you will play a vital role in supporting families with children (0-19 years) with social communication difficulties and complex health needs.
Key Responsibilities:- Deliver specialist SEND guidance, emotional support, and advice to children and young people with additional needs and their families.
- Signpost and refer to relevant services, such as earlybird training, local support groups, and parent forums.
- Work in partnership with Health Education, Social Services, and Voluntary Services to provide a holistic approach in communicating and sharing advice, information, and plans to all relevant parties and families.
- NNEB Diploma or equivalent NVQ L3 Child Care.
- Experience in working in a health setting.
- Excellent administration skills and ability to work with other professionals and voluntary and statutory agencies.
- Ability to prioritize and plan workload, evaluate work under supervision, and work on own initiative.
