Job Summary:

CWP is seeking a skilled Mental Health Practitioner to join our Home Treatment Team in Congleton. As a key member of our Urgent Care Mental Health First Response Services, you will work across various teams, including Crisis Resolution Home Treatment Teams, Liaison Services, Street Triage Services, Liaison and Diversion Services, and the Crisis Triage and Response Service.

Key Responsibilities:

  • Provide high-quality mental health care to individuals in need, working closely with our multidisciplinary teams.
  • Develop and implement effective treatment plans, taking into account the unique needs and circumstances of each client.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and excellent patient outcomes.
  • Participate in regular team meetings and contribute to the development of our services, sharing your expertise and ideas to drive continuous improvement.
  • Stay up-to-date with the latest research, best practices, and professional developments in mental health, applying this knowledge to enhance your practice and contribute to the growth of our team.

Requirements:

  • Recognized qualification in mental health, such as RMN, AHP, Psychology, or Social Care (AMHPs).
  • Evidence of further professional development involving serious mental illness.
  • Non-medical Prescribing certificate.
  • Sound knowledge of current professional staff development and continuous service improvement issues, particularly in the context of ADHD and personality disorder services.
