Staff Nurse

We are seeking a dedicated and compassionate Band 5 Staff Nurse to join our Acute Frailty Assessment Unit. In this role, you will be responsible for providing high-quality nursing care to elderly and frail patients, working as part of a multidisciplinary team to assess, plan, implement, and evaluate patient care.
Clinical Practice- Develop, implement, and evaluate personalized care plans for frail elderly patients.
- Ensure compliance with infection control policies and maintain a safe environment for patients and staff.
- Provide compassionate, person-centred care that promotes dignity, independence, and well-being.
- Communicate effectively with patients, families, and carers, providing education and support about care and treatment.
- Work collaboratively with the multidisciplinary team, including doctors, therapists, social workers, and other healthcare professionals.
- Contribute to effective handovers and ensure continuity of care.
- Participate in team meetings and quality improvement initiatives.
- Mentor and support junior staff, students, and newly qualified nurses.
- Contribute to discharge planning and ensure safe transitions of care.
- Complete risk assessments and incident reports as required.
- Participate in audits and data collection for service improvement.
- Maintain registration with the Nursing and Midwifery Council (NMC).
- Engage in continuous professional development, keeping up-to-date with current best practices in frailty care.
- Participate in mandatory training and seek opportunities for specialized learning in geriatric care.
- Contribute to the development of policies and procedures within the unit.
- Adhere to all relevant NHS and Trust policies, procedures, and guidelines.
- Participate in clinical governance activities and contribute to maintaining high standards of care.
- Support the implementation of evidence-based practice and quality improvement initiatives.
