About the Role

We are seeking an experienced Healthcare Assistant to join our Primary Care Network team. The successful candidate will have a strong background in phlebotomy and a passion for delivering high-quality patient care.

Key Responsibilities
  • Support practices in the Network to achieve QOF, LES, and DES targets for patients with complex health needs.
  • Visit housebound and care home patients for routine monitoring and provide phlebotomy services as required.
  • Work collaboratively with the multi-disciplinary general practice team to meet the needs of patients.
  • Input data to clinical templates and escalate when appropriate to the multidisciplinary team.
Requirements
  • Care certificate - Healthcare qualification (level 3 or 4) or working towards gaining equivalent level.
  • Experience of working in a primary care environment/healthcare setting.
  • Phlebotomy certification, training, and experience.
  • Immunisation training, certification, and experience.
